COVID-19 Notice: As a technology based initiative, Well Elder-Routinify continues to operate through the pandemic.

Well Elder-Routinify

The Well Elder program was developed at the beginning of COVID-19 to keep aging adults connected and informed. This Adams County community partner shared initiative, provides aging adults with the WellAssist tablet and Wearable watch through our partnership with Routinify.

Components of the program:

  • Technical support and training
  • WellAssist tablet and Wearable
  • Telecare/Telehealth provider access
  • Scheduled daily reminders
  • Increased virtual connection with family and friends

Our commitment is to provide seniors with access to resources alongside safe, trusted information from community resources.

To qualify for the WellAssist Tablet program, seniors must meet the following criteria:

  • Aged 60+
  • Reside in Adams County
